IMMUNIZATIONS

Prior to enrollment all children ages 6 weeks through 6 years must receive a physical and be
up-to-date on all required immunizations.

The parent/guardian of a school age child must complete our School Age Consent form. Signing this form indicates that the child is in good health and that he/she can participate in the normal activities of the program. In addition, it specifies any conditions or specific needs that may require special accommodations.
